NUR 358 - Patient and Population Care Management

RN license or with permission of instructor.

Description:

This course will examine the roles of case and care management in the health care system. Health promotion and disease prevention theories, frameworks, and strategies that are utilized, along with evidence-based and best practice models for intra-professional and interprofessional communication and collaboration, patient experience, population health and cost effectiveness will be presented and explored. Health care policy, advancements, and ethics within the field of case and care management will be discussed.
